Thank You for considering Sight & Sound Entertainment for your upcoming event.  The information you provide below will assist us in determining our availability, and providing you with the cost of our services.  This form is NOT a contract, and does not obligate you in any way.  We understand that you may not be able to answer all of the questions at this time, so just answer the ones you can.

  When you are finished, click on the "SUBMIT" button located at the bottom of the form.  Once we receive your event information, will will e-mail you a reply, and if available, a Services Proposal.  Unless special arrangements have been made in advance, event dates are not confirmed, nor is the client obligated, until the Services Proposal has been mailed back to our office with the required deposit.   If you have any questions, please feel free to call us at (301) 440-2778 -or- 1-800-318-0879.

Helpful Hints:

If the event is a Wedding Anniversary Party, attempt to find out what the couple's "First Dance" song was, on the day of their wedding.  List the song title and aritst in the "notes" section of this form, and the DJ will attempt to locate it, if we do not already have it in our music library.

Likewise, if there are any other "special" songs which you feel the Guest (or Guests) of Honor would like to hear, list them in the "notes" section as well, so that the DJ will be reminded to play them during the party.
